Most formal returns we see start with ward submissions that district offices forward without enough context. Provincial reviewers then ask questions the ward compiler could have answered in a footnote.

Use one percentage base per column

Mixing "percent of target" and "percent change from last quarter" in adjacent columns without clear headers causes arithmetic disputes. Ward sheets should label the base in the column header, not only in a cover note.

Footnote late entries and estimate revisions

When a ward revises a figure after initial submission, footnote the date and reason: late clinic report, corrected birth registration batch, amended contract milestone. Silent revisions break district roll-ups.

Variance wording templates

Provide ward staff three approved phrases for common situations: target not met due to seasonal demand, target exceeded due to outreach campaign, figure unchanged because activity paused. Consistent wording speeds district consolidation.

District checklist before forward

District offices should verify ward totals sum correctly, names match the official ward list, and empty cells are explained as zero activity or data not available — not left blank.
